Overall Success Rate
Vasectomy has a failure rate of approximately 1 in 2,000 to 1 in 10,000 depending on the study and follow-up duration. The AUA (American Urological Association) cites an overall failure rate of about 0.15%. This makes it more effective than tubal ligation, IUDs, and every other form of contraception except abstinence.
Early vs Late Failure
- Early failure (within first few months): Occurs when sperm are found on the first PVSA. Usually caused by recanalization — the vas deferens spontaneously reconnecting. Rate is approximately 0.5–1%.
- Late failure (after confirmed azoospermia): Extremely rare (1 in 2,000 to 1 in 10,000). May occur years after the procedure due to late recanalization.
- Technical failure: Surgeon error such as cutting only one vas or inadequate sealing. Minimized by choosing an experienced provider.
Semen Analysis Protocol
Current AUA guidelines recommend the first PVSA at a minimum of 8–12 weeks post-vasectomy, after at least 20 ejaculations. The sample can often be collected at home and dropped off at a lab — many Nashville labs offer convenient collection kits. Results typically return within 3–5 business days.
- If zero sperm (azoospermia): You're cleared. No further testing needed in most cases.

Do not skip this step
